Creating an Account for Your Family
 
  Please follow these instructions:

  • Go to “Create an Account” and fill out information for your first child.
  • Provide and submit all required information.                
  • You will know you are finished when you reach the confirmation message.  If this is your only child, then you have completed the registration process.
If you have more than one child, you can create a "Family Account":
  • The first account you created can be designated as the "Primary Account".  After filling out the forms for the first child, go to "My Patient Page" and click on "Change My Family Account" in the "My Account" box.
  • To create another account for the second child, click on "Create a Family Member" under "My Family Account".  Then go through the same process.
  • Link the new account to the "primary account" by clicking on "Link an Existing Family Member".
  • Repeat this process for every child in your family who will be seen in our office.
  • The next time you log in, using  the ID/password that you created for the first/primary account, you will be able to specify which child/patient account you want to bring up by choosing it in the dropdown box where it says “Active Account.”
